We’re thrilled that you’re interested in joining us for the 2025 NFBI Chicago Chapter Bowling Fundraiser! To help us ensure a smooth and enjoyable event, please complete and submit this registration form by midnight on August 11, 2025. If you plan to register multiple attendees, please fill out a separate form for each individual to help us keep track accurately.

Please note that all form fields must be completed and payment must be submitted in order to finalize your registration. If you prefer not to pay online, you may pay in person at the event by locating either Kira O’Bradovich, NFBI Chicago Chapter Treasurer, or Marilyn Green, NFBI Chicago Chapter Board Member, during the fundraiser on Saturday, August 16, 2025, from 4:00 to 6:00 pm.
To help make your arrival at the bowling alley quicker and easier, we encourage you to complete your payment online before the event. This way, you can spend more time enjoying the fundraiser and less time waiting in line.
